The only way I have ever heard of being able to accomplish this is to turn the PDF file into an image (e.g. bitmap etc.., you can use Ghostscript to do this) then analyse each pixel for it colour ( you can do this using System.Drawing).
If there is an easier way then I would also be very interested to hear it, as the above method is a rather large chore. I know that PDFsharp was originally based on GDI+ so this may indeed be possible (although I have not noticed it).
